Report a data problem

If a number on this site doesn't match whatMedicare Care Compare shows for the same facility, we want to know. Include the facility's CCN (shown at the bottom of every facility page) and which number looks wrong; we check every report against the CMS source data. Note that CMS updates monthly, so a recent real-world change may not appear in either place yet — each facility page shows the date of the record it displays.

If you have an urgent concern about a nursing home

This site is a records archive — it can't act on complaints. These offices can:

• Immediate danger: call 911.
• Suspected abuse or neglect: your state's Adult Protective Services, and the facility's state survey agency (every state runs a complaint line — find yours via Care Compare or your state health department site).
• Resident-rights problems, quality concerns: the Long-Term Care Ombudsman for your area — a free, independent advocate for residents.
